Assignment

The approved amount agreed upon by medicare and your doctor or medical provider for the services or supplies provided to you. This is the full amount they will charge.

Coinsurance

Coinsurance is a set percentage you pay towards your health care. This is after your deductible has been met.

Custodial Care

Coverage for care not performed by medical professionals. This means people who do not have a nursing or medical professional training backgrounds.

Extra Help Program

Beneficiaries who are receiving low-income subsidy qualify for the extra help program. These beneficiaries receive assistance in paying for their part D monthly premium, annual deductible, coinsurance, and co-payments. These individuals also do not have any gap in prescription coverage, known as the “donuts” hole.

Lifetime Reserve Days

Each individual has 60 non-renewable extra days for which medicare will pay over the original 90 covered days. For example, if you stay in the hospital for 110 days during your benefit period, Medicare will pay for 90 of those days. The 20 additional days can be taken out of your lifetime reserve days. Usage of these 20 days would reduce to the reserve to 40 days. Once all 60 days have been used, the beneficiary will not receive any extra days during their lifetime.

Lifetime Reserve days are covered fully by medicare besides coinsurance.

Medicare-Approved Amount

Fee that health insurance plan sets as the amount a provider should be paid for a particular service.

Primary Care Doctor

Your physician you first turn to when any health related care issue arises. These physicians can refer you to any specialist for extra care.

Benefit Period

Time spent in the hospital or skilled nursing facility.

Co-payment

A set amount that you pay each time for a service or prescription from the doctor.

Deductible

Your deductible is the amount you pay for one year in services or prescriptions before coverage begins.

Formulary

A formularys is a list of covered drugs in your health insurance plan. These drugs are FDA approved and are placed in different tier groups based on coverage level. The amount you have to pay is based upon the tier level and the plan you choose.

Medically Necessary

Services or supplies needed to help an individual prevent, treat or diagnose an illness, condition or disease.

Service Area

Each plan is designated to certain areas of coverage. If you move out of this area, you will be disqualified from that plan.
